Rectal Pelvic Pain From Scar Tissue From 1st Birth

Has anyone else experienced the excrutiating pain of passing bowels with episiotomy scar tissue? My Dr. said there's nothing to do about it but to keep regular, drink lots of water, and exercise. It is tearfully painful. Does anyone else have any experience with this, or any advice?
