The sac is normal, but there's something else that is too big to be a twin. I'm only in my 4th week... What could it be? Can a pregnancy continue normally?

Your are probably seeing a corpus luteum cyst. The corpus luteum is the follice that you ovulated from. It supports the embryo until the placenta is formed. In a small percentage of women it can turn into a cyst instead of shrinking like it normally does. Usually it will go away on its own, but doctors usually keep an eye on it during pregnancy in case it should twist or rupture. It won't hurt the pregnancy, but it can hurt.
