One day I put my little girl in her exersaucer and she was literally 2 feet away from me while I was doing dishes. All of a sudden a hear a loud scream, I turned around and I saw my little munchkin had the holder for the pacifier in her mouth. I don't know how she managed that, but the entire thing was in her mouth and she was trying to suck it (probably thinking it was her pacifier). She couldn't breathe and was getting panicked. Then, when I was trying to get it out of her mouth I noticed that the metal part was on her gums. It cut her little gums a bit and my poor baby was crying so hard, and of course I was crying with her. After I took it out I was just holding her and we were both crying our little heads off!!! It was so traumatizing. I really have no idea how she managed that. Needless to say, I refuse to use those pacifier holders now!

Wow, I'm glad your little one is okay also. It's amazing how something can happen so quickly. I think, if I could, I would put my Nathan in a bubble (just kidding). What kind of pacifier holder was this?
