Neg HPT But Feel Pregnant

Here's my problem, My husband and I have been married a year and a half and I have been off the pill all that time. I think we had one early miscarriage last year. I have always had really irregular peiods, only the pill regulated them, but since I've been off it, it's not unusual for me to skip a month or even 2. It has now been nearly 4 months since I had one. I have been really tired, and even though I have been making changes in my eating and lifestyle to lose weight, I've gained 12 pounds. I also started feeling what felt like butterflies in my tummy, so I took a HPT, and it was negative, so I thought that was that. I still felt pregnant though and the butterflies haven't stopped, I feel them throughout the day, and especially if I wear any tight fitting pants or lean my stomach against something. I was just into the Dr. to get a blood test, and he seemed to almost not want to give me one, saying it was highly unusual to have a neg HPT but still be pregnant. He had that look like he was just humoring me by doing the test. I talked to my sister, and she said both she and my mother had a similar problem with negative tests when they were really pregnant and had to get sonograms done to find out they were 3months + along. If the blood test comes back negative, should I push for a sonogram, or is it pretty definative? I think maybe I should get a sonogram anyway, just to find out what is going on, if I've been 4 months without a period, something is wrong somewhere right?? I'm 28 years old.
