Does This Mean Im Having Twins

this is my first pregnancy. im at 5 months and have gain 24 lbs. many people who have had babies tell me that my stomache is so big at 5 months like im having twins. when i was at the 2nd month my stomach was already showing too. i went to do a ultrasound and it was a boy. in two of the pictures it show's my baby boy's face and next to it looks like a rounded circle like the top of another baby's head. if i am having twins shouldn't the doctors recognize it right away?.
