My Sister Is Leaking Fluids Should She Be Concerned

My sister is 5 weeks away from her due date. She is 2 cinameters dialated and completely thinned out. She noticed this morning she is leaking fluids. She called her doc and they said her water is not breaking yet. What causes this? If anyone will respond then I will appreciate it.
