yes, I do. Sometimes several because the wet diaper is up against their skin and can be irritating and may also lead to rashes, so I also use a lot of desitin.

I definitely use a wipe. At one time I wasn't and my son developed a yeast rash. So now one wipe for EVERY wet diaper. Once every two weeks I'll put vaseline all over the diaper area because sometimes my son gets red patches on certain areas of his lil boy parts (I'm sure from the diaper rubbing and the wetness). Vaseline was the only thing that worked 100% for my son, I dunno why.

i do for my son, but with a little girl i watch, i don't. She gets a very bad rash because she has super sensitive skin, and overwiping it makes it raw.. about every third wet diaper i use one just to make her clean, but the rest of the time i let her dry out a bit and just put the new diaper on.. Her parents asked me to do this after her doctor suggested it, and she has had little problems since.

not every time. i usually wipe well in the morning and before bedtime, and then play it by ear during the day. often, i leave his diaper off for a while as well, so he can air himself out. he has never had a diaper rash, so i think it works well!
